At this time we are open for appointment-based services. At Black Creek CHC, we remain committed to keeping our services accessible, with measures in place to protect the health and wellbeing of our community, staff and partners during the pandemic. Pending new directives from public health authorities, this is what you can expect from Black Creek CHC now and in the coming months: • Our locations are open Monday to Friday, 9:00 am – 5:00 pm for appointment-based services • Active screening will continue for all clients and visitors to the centre and contact information will be collected for contact tracing • Remote support is available through phone/videos appointments to reduce unnecessary in-person visits and travel • Harm Reduction supplies including condoms and safe supply kits are available for pick-up at our Sheridan site on Mondays – Fridays from 2:30 – 4:30 • Continued distribution of harm reduction supplies, masks, food and other household supplies to vulnerable residents, while practicing safety measures When visiting, clients are reminded that they need to: • Wear a mask, stay 6 feet apart from others & practice good hand hygiene • Come alone to your appointment, if you are able to • Provide answers to our screening questions and provide your contact information
